Come learn about the new Medicaid Alternative Payment Model for primary care from Medicaid and CAFP staff. The new APM has a focus on care for chronic conditions and practice transformation, plus a 16% reimbursement increase paid through partial capitations, allowing providers to direct funds to where their patients need it most.

All primary care medical providers participating in the Accountable Care Collaborative who have 500 or more attributed Health First Colorado ACC enrollees are eligible to enroll in APM 2.0.
