CAFP Awards

These awards are limited to physicians, residents, and practices in Family Medicine. Information about each award can be found below. Honorees are celebrated each year at the CAFP Foundation Annual Summit.

Family Physician of the Year

The Family Physician of the Year Award recognizes an outstanding family physician who provides exceptional care, enhances their community, is a role model professionally and personally, and is an exemplary representative of the AAFP, CAFP, and family medicine profession. Include information such as:

  • Accomplishments and activities
  • Exceptional qualities that position them as a leader among family physicians
  • Impact to patients, community and/or the specialty of family medicine
  • Patient Care
  • Excellence in the Profession of Family Medicine
  • One or more of the following:
    • Practice Leadership
    • Community Service and Contributions
    • Scholarly Activity
    • Advocacy
  • Experience and Career Accomplishments 
  • Commitment to the principles of diversity, equity, and inclusion

Family Medicine Teacher of the Year 

The Family Medicine Teacher of the Year Award recognizes an exceptional family medicine educator, teaching part-time, full-time, or volunteer. Nominees should demonstrate dedication to preparing the next generation of family physicians to provide excellent, patient-centered care. Include information such as:

  • Current practice and teaching location
  • Accomplishments and Activities
  • Impact to learners and community
  • Exceptional qualities that position them as a leader among family medicine teachers
  • Patient Care
  • Teaching Commitment 
  • Excellence in Teaching
  • Commitment to the principles of diversity, equity, and inclusion

Family Medicine Resident of the Year

The Family Medicine Resident of the Year Award recognizes a first, second or third-year resident who demonstrates clinical, academic or teaching excellence and a dedication to patients, colleagues, and the community. Include information such as:

  • Accomplishments and activities in residency
  • Qualities that position them as a leader among family medicine residents
  • Patient Care
  • Academic Excellence or Scholarly Activity
  • Community Service or Activities
  • Commitment to the principles of diversity, equity, and inclusion

Practice Award for Health Equity and Community Engagement

The Practice Award for Health Equity and Community Engagement is intended to highlight and celebrate practices in family medicine that are making progress in reducing racial and other health disparities. The practice receiving the award will receive $1,000 from the CAFP Foundation. Please include the following in your nomination:

  • Describe projects, systems, or other activities the practice has implemented to reduce health disparities, build community, or advance health equity.
  • Describe how these reduced or shows promise to reduce health disparities for the practice’s patients.
  • Describe how these have impacted the practice’s team.
  • Share any data and supplemental materials available demonstrating the success, anticipated success, or lessons learned.

The Colorado Academy of Family Physicians (CAFP) reserves the right to share information about the practice’s projects/activities to address health equity and community engagement. This can include the practice name, team members, and details about the projects/activities. The sharing of this information is intended to promote the practice’s work and inform others of practical activities they can undertake to improve health equity and community engagement.
